Here’s what to expect after submitting the form:
Initial Contact During Business Hours:
A member of our team will reach out to you during business hours (Monday–Friday, 9am–5pm), using your preferred contact method. This first call is to learn more about your goals, collect some basic health information, and set you up in our secure online system (Jane App).Welcome Email & Intake Forms:
After your profile is created, you’ll receive a welcome email with your intake and informed consent forms. These must be completed before your first appointment. If you don’t use email or have trouble with online forms, your practitioner can assist you at your initial visit or bring printed copies.Practitioner Match & Appointment Scheduling:
Once you're set up, we’ll match you with a practitioner who serves your area. They’ll contact you directly to introduce themselves and schedule your in-home initial assessment.
We aim to schedule your assessment within 24–72 hours of your setup, depending on practitioner availability in your area.
